To register for the WASFAA Spring 2009 Conference, being held April 6-8,in Madison, please complete the form below.  A confirmation page and invoice will be generated for you to print.

Conference Registration Fees:
$100 for members
$170 for non-members (annual membership is included with non-member full conference registration)
$ 60 one-day registration is available to WASFAA members (Monday, Tuesday or Wednesday).
(One-day registration fee is not available to non-members)
$180 on-site registration

We expect and appreciate your payment by March 20th, 2009 for conference registration purposes so please plan according to your institution’s payment process!
